PRN - Physical Therapist, Peridot


At least 6 months previous clinical experience.
Bachelor’s degree in Physical Therapy or equivalent combination of education and experience; Master’s or Doctorate degree preferred
Current State of Arizona licensure as a Physical Therapist
Current BLS certificate or the ability to obtain within 30 days

Provides direct patient care to individuals with physical disabilities and/or functional limitations in a clinic, inpatient, outpatient and/or outreach program/setting

ESSENTIAL FUNCTIONS (include, but not limited to the following)

 Develop plan of care for patients with neuromusculoskeletal disorders based on and in collaboration with physician orders.

Administers appropriate physical agents within physician protocols, patient’s medical history and therapist’s knowledge of indications and contra-indications including Electrotherapy, sound energy, heat/cold therapy, hydrotherapy and JOBST compression
Prescribes exercise plan and provides demonstration, instruction, and cues for active and passive exercises, muscle reeducation, gait/functional training; performs therapeutic exercise with and without equipment
Adapts conventional techniques to meet patient needs; educates patients and family members
Performs manual therapy techniques utilizing hands on and tool assisted techniques with understanding of indications and contraindications and precautions and including, but not limited to: soft tissue mobilization, extremity and joint mobilization, medical exercise therapy, myofascial release and craniosacral therapy
Establishes plan of care and documents progress, treatments, assessment, and discharge summary.
Maintains equipment through implementation of preventive maintenance and repair
Evaluates, records and reports on patient’s progress for review by other members of the multidisciplinary treatment team
Develops regularly schedule and not scheduled in-services for sharing knowledge and information and techniques with other clinicans
Instructs patients and families in appropriate home exercise programs.
Participates in departmental orientation, on the job training and quality assurance programs/initiatives
Participates in a variety of department and hospital educational programs to maintain current skills and competency levels; identifies and discusses performance or training needs with Supervisor
Operates motor vehicle to commute between clinic settings as well as primary hospital campus
Performs all other job related activities as requested
