Two years of recent experience planning regular and modified diets in a public institution
Two years of clinical nutrition work experience with specific emphasis on providing medical nutrition therapy targeting chronic diseases like diabetes, heart disease, etc., and nutrition education throughout the life cycle, especially geriatric nutrition
Bachelor's Degree in Dietetics from an accredited college or university
Completed a Didactic Program in Dietetics Accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
Current registration with the Academy of Nutrition and Dietetics

The Registered Dietitian provides Medical Nutrition Therapy (MNT) for inpatients and outpatients served by the San Carlos Apache Healthcare Center. The MNT includes assessment, nutrition care planning using the Nutrition Care Process Model, counseling and education, as well as implementation of special therapies, such as medical and enteral foods.

ESSENTIAL FUNCTIONS (include, but not limited to the following)

Provides MNT for inpatients and outpatients served by SCAHC. MNT includes performing assessments, creating nutrition care plans, counseling and education, and administration of special therapies, such as medical foods.

Serves as a Subject Matter Expert in nutrition, overseeing the medical nutrition therapy of the complex and high-risk patients.
Provides in-service education and/or training, education programs to nursing staff, medical staff, students, healthcare personnel and the general public, as needed.
Modifies patient computer profiles, in accordance with nutritional need, food intolerances and food preferences.
Participates as a member of the medical team at interdisciplinary care conferences to develop and/or update a patient care plan.
Provides MNT for adults and/or family members referred by a physician for outpatient services. MNT includes performing assessments, creating nutrition care planning, providing individualized guidance on food selection and meal preparation for disease management and prevention of complications, and follow-up as needed.
Performs regular food service department quality assurance checks as assigned.
Provides nutrition-related presentations/information for the community.
Provides in- service education for Food Service personnel as requested.
Prepares reports and provides other documents needed to maintain accreditation with regulatory agencies.
Performs other duties as assigned.
